In FY10, BHI is continuing its efforts towards improving coordination of care between mental and physical health providers. Below is an updated training packet that includes a flyer, the National Association of State Mental Health Programs Directors (NASMHPD) report on morbidity and mortality in people with serious mental illness, an updated PowerPoint presentation (one for clinicians and one for prescribers), and the PCP Notification of Care Letter template that should be completed for all clients.

BHI will be conducting a chart audit on mental health records from all its providers in the next year for the presence of a completed PCP Notification of Care letter and documentation (intake assessments, psychiatric/medication evaluation, and progress notes) of conversations with the client about obtaining and seeing a PCP and evidence of coordination of care between the mental and physical health providers for that client.
