Advance Directive Information
An advance directive tells your doctors what kind of care you want if something happens and you are not able to make those decisions (like a coma). For example, you might not want to be on life support if you have a condition you are not likely to recover from. Adults (age 18+) who are able to make decisions for themselves can do an advance directive.

Advance directives:

  • Protect your right to make medical decisions about your care
  • Help family members make decisions if you are not able to
  • Help your doctors by telling them your wishes
